Ozanimod Hydrochloride falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Guarana interacts with.

Theoretically, concomitant use might increase the risk of a hypertensive crisis.
Guarana contains caffeine. Caffeine has been shown to inhibit MAO-A and -B in laboratory studies. Concomitant intake of large amounts of caffeine with MAOIs might precipitate a hypertensive crisis. In a case report, a patient that consumed 10-12 cups of caffeinated coffee and took the MAOI tranylcypromine presented with severe hypertension. Hypertension was resolved after the patient switched to drinking decaffeinated coffee.

Here's the connection with Zeposia (ozanimod). One of the breakdown products of ozanimod gently blocks an enzyme called MAO. Guarana is basically a concentrated source of caffeine, and in lab studies caffeine can also nudge that same enzyme. The worry on file is mostly theoretical, but in large amounts the combination could push your blood pressure up more than expected. That matters because ozanimod can already affect heart rate and blood pressure, especially when you first start it.

A little caffeine from food or a normal cup of coffee is unlikely to be a problem. The concern is with strong, repeated guarana doses (think energy products or big supplement amounts). If you get headaches, a pounding heart, chest discomfort, or your blood pressure reads high, that's worth attention. Please check with your pharmacist or doctor before adding guarana.
