Hemp and Clomipramine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Hemp interacts with whole categories of medication. Clomipramine falls into 2 of those categories, so it can interact with Hemp in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Clomipramine

Clomipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hemp interacts with.

Theoretically, hemp might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that hemp induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body uses a liver enzyme called CYP1A2 to help break down clomipramine. Lab studies suggest hemp may speed that enzyme up a bit. If that happened in real life, your body could clear clomipramine a little faster, which in theory might lower its level and make it work less well for your OCD symptoms.

I want to be honest about how thin this evidence is. This comes only from test-tube research, the rating is minor, and eating hemp seeds or hemp oil as food is very unlikely to change anything. Concentrated hemp supplements are more of a question mark. Since clomipramine takes time to work and dose matters, don't change anything on your own. Just give your pharmacist or prescriber a heads-up before starting a hemp supplement.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Clomipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Clomipramine (Anafranil): this medicine is partly broken down by a liver enzyme called CYP3A4. Some early lab research hints that hemp may speed that enzyme up. If that happened in the body, your clomipramine could clear a little faster, which in theory might leave less of it around and make it work less well for your OCD symptoms.

I want to be honest with you though, this is only test-tube evidence, and it's rated a minor, possible interaction. Eating hemp seeds or hemp oil as food is very unlikely to cause a real problem. If you're using concentrated hemp supplements daily, just mention it to your pharmacist or doctor so they can keep an eye on how well your clomipramine is working. Don't change anything on your own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Clomipramine page

“A tricyclic antidepressant used mostly for OCD, clomipramine (Anafranil) is unforgiving of shifts in its blood level, which is why its 236 supplement interactions deserve genuine wariness. Most of the list involves metabolism: herbs that speed up or slow down the liver enzymes handling clomipramine can drag its level too low or push it…”

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.
