Why this applies to Selpercatinib
Selpercatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hemp interacts with.
In vitro research shows that hemp induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this pairing. Selpercatinib (Retevmo) is processed in your body by an enzyme called CYP3A4. Some early lab research suggests hemp might speed that enzyme up. If that happened, your body could clear selpercatinib a little faster, which in theory might lower its blood levels and make this cancer medicine work less well over time. Selpercatinib is not a prodrug, so faster breakdown means less drug, not more.
I want to be honest about the strength of this. The signal comes only from test-tube studies, not from people, so we really don't know if it matters in real life. Still, because this is a cancer treatment where keeping steady levels counts, please check with your oncologist or pharmacist before adding any concentrated hemp supplement.

What Is Selpercatinib Used For?
Brand name: Retevmo
Selpercatinib, sold as Retevmo, treats cancers driven by a change in the RET gene. Your tumor must test positive for the change using an FDA-approved test.
Retevmo is used for:
- Adults with locally advanced or metastatic non-small cell lung cancer (NSCLC) with a RET gene fusion
- Adults and children 2 years and older with advanced or metastatic medullary thyroid cancer (MTC) with a RET mutation who need systemic therapy
- Adults and children 2 years and older with advanced or metastatic thyroid cancer with a RET gene fusion who need systemic therapy and are radioactive iodine-refractory (if radioactive iodine is appropriate)
- Adults and children 2 years and older with locally advanced or metastatic solid tumors with a RET gene fusion that have progressed after prior treatment or have no satisfactory alternative options
