These two interact in 4 distinct ways. Each one is explained separately below.
Why Are There 4 Interactions?
Yohimbe interacts with whole categories of medication. Acetaminophen, Aspirin, Caffeine falls into 4 of those categories, so it can interact with Yohimbe in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
STIMULANT DRUGS
1 of 4Why this applies to Acetaminophen, Aspirin, Caffeine
Acetaminophen, Aspirin, Caffeine falls under STIMULANT DRUGS — the group Yohimbe interacts with.
Yohimbine, a constituent of yohimbe, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. Theoretically, taking yohimbe with stimulant drugs can have additive stimulant and hypertensive effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing with a product like Excedrin: the part that matters for this combo is the caffeine. Caffeine is a stimulant, and yohimbe (through its active compound yohimbine) is also a stimulant that can raise blood pressure and heart rate. Stack the two together and those effects can add up.
What might you feel? Things like a racing or pounding heart, jitteriness, anxiety, trouble sleeping, and a bump in blood pressure. The acetaminophen and aspirin parts aren't really the concern here. I'll be honest, this is a theoretical interaction based on how both work, not strong study data, so it's not a reason to panic. Still, if you have high blood pressure or heart issues, check with your pharmacist or doctor before combining them.

ANTICOAGULANT/ANTIPLATELET DRUGS
2 of 4Acetaminophen, Aspirin, Caffeine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the connection with this combo product, like Excedrin: the aspirin part already thins your blood a little by making platelets less sticky. Yohimbe, in lab studies, seems to nudge platelets in the same direction. So in theory, taking them together could add up and make it slightly easier to bruise or bleed, or take a touch longer to stop bleeding from a small cut.
That said, I want to keep this in perspective. This is a minor, theoretical concern based only on test-tube and animal research, not on real-world cases. The acetaminophen and caffeine pieces aren't part of this at all. If you take aspirin daily, are on a blood thinner, or have a bleeding condition, just run it by your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
3 of 4Acetaminophen, Aspirin, Caffeine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research shows that yohimbe extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
In a product like Excedrin, the part that interacts here is the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2, and lab studies suggest yohimbe might speed that enzyme up a bit. If that happened in real life, your body could clear caffeine a little faster, meaning the caffeine 'boost' in Excedrin might not last as long. The acetaminophen and aspirin parts aren't really affected by this.
Honestly, this is a theoretical concern based only on test-tube research, and it's rated minor, so I wouldn't lose sleep over it. The bigger issue is yohimbe itself, since it can raise blood pressure and heart rate, and caffeine can add to that jittery, racing-heart feeling. If you take both, check in with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
4 of 4Acetaminophen, Aspirin, Caffeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture for an Excedrin-type product (acetaminophen, aspirin, and caffeine). The concern on file is that yohimbe may speed up a liver enzyme called CYP3A4, which could clear certain medicines a bit faster. But this is based only on lab dish studies, not on real people, so it's very much a theoretical note rather than a proven problem.
Even if it did happen, the pieces of Excedrin don't lean heavily on CYP3A4, so I wouldn't expect your headache relief to noticeably change. What worries me more is the overlap in effects: yohimbe and the caffeine here can both raise your heart rate and blood pressure, leaving you jittery or anxious. If you take blood pressure or heart medicines, please run this combo by your pharmacist first.

About Yohimbe
What is Yohimbe?
- Erectile dysfunction
- Low sex drive
- Sexual performance
- Weight loss
- Athletic performance
Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
