Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.

Here's the basic idea with these two together. Your body uses an enzyme called CYP3A4 to break down bedaquiline (Sirturo). Lab studies suggest yohimbe may speed that enzyme up. If that happened in real life, your body could clear bedaquiline a little faster, which in theory might lower its levels and make it less effective against your drug-resistant TB. With a serious infection like MDR-TB, you really don't want anything chipping away at how well your medicine works.

That said, this concern comes only from test-tube research, not from people, so it's rated as minor and theoretical. Honestly, yohimbe carries its own risks like high blood pressure and a racing heart, which I'd worry about more here. Please check with your pharmacist or doctor before mixing them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Bedaquiline

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
