This combo cold product pairs an antihistamine (chlorpheniramine) with a cough suppressant (dextromethorphan). Dextromethorphan is cleared mainly by an enzyme called CYP2D6. Lab studies hint that yohimbine, the active part of yohimbe, may slow that enzyme down. If it does, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or feeling foggy. Neither ingredient is a prodrug here, so blocking the enzyme would raise levels rather than weaken the effect.

Keep in mind this is only a theoretical concern based on test-tube research, not real people, so I wouldn't lose sleep over it. Still, yohimbe on its own can raise blood pressure and heart rate, so check with your pharmacist before combining them.

In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..

Here's the thing with this combo cough and cold product: both chlorpheniramine and dextromethorphan can gently slow down a liver enzyme called CYP2D6. Yohimbine, the active piece of yohimbe, happens to lean on that same enzyme to get cleared from your body. So when you take them together, yohimbine can build up higher than it normally would.

Why does that matter? Yohimbine is the part of yohimbe most likely to cause trouble, things like a racing heart, jitteriness, anxiety, and spikes in blood pressure. Higher levels mean those effects can hit harder. This is a real, documented effect (seen with other CYP2D6 blockers), though most data come from prescription yohimbine, not the unpredictable over-the-counter bark. Given yohimbe's safety concerns on its own, I'd run this pairing by your pharmacist or doctor first.

Here's the gist for this cough-and-cold combo. Lab studies suggest yohimbe may speed up a liver enzyme called CYP3A4, which helps process the dextromethorphan (the cough suppressant) part of this product. In theory, that faster clearance could make the cough relief a little weaker. Honestly though, this is based only on test-tube research, so we don't really know if it matters in real people. The chlorpheniramine (the antihistamine) isn't the main concern here.

My bigger worry is overlap in side effects. Both yohimbe and this medicine can raise your heart rate and blood pressure, so combining them may leave you jittery, anxious, or with a racing heart. I'd run it past your pharmacist or doctor before pairing them.
