Yohimbe and Dronedarone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Yohimbe interacts with whole categories of medication. Dronedarone falls into 3 of those categories, so it can interact with Yohimbe in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

1 of 3

Why this applies to Dronedarone

Dronedarone falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the thing to know about pairing yohimbe with dronedarone (Multaq). Dronedarone blocks a liver enzyme called CYP3A4, and that same enzyme is what your body uses to clear yohimbine, the active piece of yohimbe. So when both are on board, yohimbine may be broken down more slowly and build up higher than expected.

The worry is that yohimbe's own side effects, like a racing heart, palpitations, and spikes in blood pressure, could become stronger. That's especially concerning here, because you're taking dronedarone to keep an irregular heart rhythm in check, and yohimbe can work against that goal. This warning is theoretical (based on how the drugs behave, not hard outcome studies), but given the heart stakes I'd run it past your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS

2 of 3

Dronedarone also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing yohimbe with dronedarone (Multaq). Dronedarone blocks a liver enzyme called CYP2D6, and that enzyme is exactly what your body uses to clear yohimbine, the active piece of yohimbe. When that pathway is slowed down, yohimbine can pile up higher than it normally would.

The worry isn't really about dronedarone weakening. It's that the extra yohimbine can push its own side effects, things like a racing heart, jumpy or pounding heartbeats, rising blood pressure, and anxiety. That's not ideal for someone managing a heart rhythm problem in the first place. This is a probable interaction with reasonable evidence behind it, so please check with your pharmacist or doctor before combining the two.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Dronedarone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your Multaq (dronedarone). Your body relies on a liver enzyme called CYP3A4 to break this medicine down. Some early lab research suggests yohimbe may speed that enzyme up, which could clear dronedarone out a little faster and leave you with lower levels. In plain terms, your heart-rhythm medicine might not work quite as well, which is a concern since the whole point is keeping you out of atrial fibrillation and out of the hospital.

I want to be honest though: this is theoretical, based only on test-tube studies, and rated minor. But yohimbe carries its own real risks of high blood pressure and a racing heart, which is the last thing you want with a heart condition. Please run it by your pharmacist or doctor first.

About Yohimbe

What is Yohimbe?

Yohimbe
  • Erectile dysfunction
  • Low sex drive
  • Sexual performance
  • Weight loss
  • Athletic performance

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Dronedarone

What Is Dronedarone Used For?

Brand name: Multaq

Multaq (dronedarone) tablets are used to reduce the risk of being hospitalized for atrial fibrillation (AF) — an irregular, often rapid heart rhythm. It is approved for adults who are currently in normal sinus rhythm but have a history of paroxysmal (comes and goes) or persistent atrial fibrillation.

Multaq is not appropriate for everyone with AF. It must not be used in people with permanent atrial fibrillation or in those with recently worsened heart failure requiring hospitalization — these patients face a doubled risk of death, stroke, and hospitalization if they take it.
