Theoretically, yohimbe might reduce the effects of antihypertensive drugs.
Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.

Here's the thing to know about riociguat (Adempas): it works by relaxing and widening blood vessels, which lowers the high pressure in your lung arteries. Yohimbe pulls in the opposite direction. The yohimbine in it can raise blood pressure and bump up your heart rate, so it may partly cancel out the benefit you're getting from riociguat.

This concern is mostly theoretical, based on how yohimbe behaves in the body rather than studies of these two together. Still, riociguat is a serious medication for a serious condition, and over-the-counter yohimbe products are unpredictable in strength. I'd play it safe and check with your doctor or pharmacist before pairing them, rather than risk working against your treatment or causing a racing heart.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.

Here's the gist for riociguat (Adempas): your liver uses an enzyme called CYP3A4 to help clear this medicine from your body. Lab studies (just test-tube research, not people) hint that yohimbe might speed up that enzyme. If that happened in real life, your body could break down riociguat a little faster, which in theory could leave you with a weaker effect on your lung blood pressure.

Honestly, this is a very theoretical, minor concern, and riociguat is not a prodrug, so I wouldn't lose sleep over it. The bigger worry is yohimbe itself, since it can raise blood pressure and speed up your heart, which works against what riociguat is trying to do. Please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold under the brand names Adempas and Riociguat — are approved for adults with two types of high blood pressure in the lungs. The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots that have blocked or narrowed the arteries in the lungs. Riociguat is used when surgery has not fully fixed the problem, or when surgery is not an option.

The second is pulmonary arterial hypertension (PAH), a progressive disease where the arteries in the lungs become stiff and narrowed. In PAH, riociguat helps improve exercise ability, functional class (how well you can get around day-to-day), and slows disease worsening. It can be used alone or combined with other PAH medicines called endothelin receptor antagonists or prostanoids.
