Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.

Here's the basic idea with yohimbe and Selinexor (Xpovio). Selinexor is broken down in your body partly by an enzyme called CYP3A4. Lab studies suggest yohimbe may speed that enzyme up, which could in theory clear Selinexor out a little faster and slightly weaken how well it works against your myeloma or lymphoma. Selinexor is not a prodrug, so faster clearance means less drug effect, not more.

I want to keep this in perspective. This is rated minor and comes only from test-tube research, not from people, so we really don't know if it matters in real life. Still, with a serious cancer drug like this, I wouldn't add yohimbe on your own. Please run it by your oncology team or pharmacist first.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Selinexor

What Is Selinexor Used For?

Brand name: Xpovio

Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.

Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
