Selumetinib Sulfate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Yohimbe interacts with.

Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Here's the honest picture for Koselugo (selumetinib) and yohimbe. The interaction on file isn't really about how your body breaks selumetinib down. Instead, yohimbe contains yohimbine, which in lab studies (at doses of 8 mg or more) seemed to make platelets a little less sticky. Selumetinib is grouped with blood thinners because it can carry a bleeding risk too, so the theory is that the two together might add up and raise the chance of bruising or bleeding.

I want to be clear: this is only a theoretical concern from test-tube data, not something seen in real patients, and it's rated minor. Still, since this medicine is for a child with NF1, I'd loop in your oncology team or pharmacist before adding any yohimbe product rather than guessing.

Here's the basic idea with yohimbe and selumetinib (Koselugo). Your child's body clears selumetinib using a liver enzyme called CYP3A4. Some early lab research suggests yohimbe may speed that enzyme up. If that happened in real life, it could clear selumetinib out faster, leaving less of it in the body and possibly making it work less well against the neurofibromas. Selumetinib is not a prodrug, so this would mean weaker effect, not stronger.

I want to be honest about how thin this evidence is. It comes only from test-tube studies, and the interaction is rated minor. Still, since this medication is fighting tumors, I'd loop in your oncology team or pharmacist before adding yohimbe, which carries its own heart and blood pressure risks.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
