Aluminum Hydroxide, Aspirin, Codeine Phosphate, Magnesium Hydroxide falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Guarana interacts with.

Theoretically, guarana may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that guarana extract can inhibit platelet aggregation. This effect may be due to the caffeine in guarana, which is also reported to have antiplatelet activity. This interaction has not been reported in humans.

This combination product already contains aspirin, which is a blood thinner that keeps platelets from clumping together. Lab and animal studies hint that guarana (mostly because of its caffeine) can also make platelets a little less sticky. Put the two together and, in theory, you could nudge your bleeding risk up slightly. That might show up as easier bruising, a nosebleed that takes longer to stop, or stomach upset on top of what aspirin can already cause.

Here's the honest part: this has only been seen in test tubes and animals, never confirmed in people, so I wouldn't lose sleep over an occasional cup. Still, if you take this medicine regularly, it's worth a quick chat with your pharmacist or doctor before adding a daily guarana supplement.
