Here's the short version: guarana is basically a strong source of caffeine, and metformin works to lower your blood sugar. The concern on file is that caffeine might nudge your blood sugar up or down, which could make it a little harder to tell how well your metformin is working. Honestly, the evidence here is mixed and weak, and this is rated a minor, unlikely interaction.

Metformin itself isn't broken down by the liver in a way caffeine would change, so guarana doesn't make metformin build up or wear off. The real-world point is mostly about your blood sugar readings. If you use guarana regularly and notice your numbers acting differently, or you feel jittery, shaky, or get a racing heart, check your sugar and mention it to your pharmacist or doctor.

Here's the thing to know about pairing guarana with metformin: guarana is basically a concentrated source of caffeine, and the concern flows in one direction. The idea is that metformin may slow down how your body clears caffeine, so the caffeine from guarana could hang around a little longer. This isn't about metformin becoming weaker or stronger. Metformin keeps doing its job on your blood sugar just fine.

I want to be honest about how soft this evidence is. It comes from animal and lab studies only, and it has never actually been reported in people. So at most, you might feel the usual too-much-caffeine stuff a bit more: jitteriness, a racing heart, trouble sleeping, or anxiety. If you have heart issues or notice those symptoms, check in with your pharmacist or doctor.
