Why this applies to Moclobemide

Moclobemide falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Guarana interacts with.

Theoretically, concomitant use might increase the risk of a hypertensive crisis.
Guarana contains caffeine. Caffeine has been shown to inhibit MAO-A and -B in laboratory studies. Concomitant intake of large amounts of caffeine with MAOIs might precipitate a hypertensive crisis. In a case report, a patient that consumed 10-12 cups of caffeinated coffee and took the MAOI tranylcypromine presented with severe hypertension. Hypertension was resolved after the patient switched to drinking decaffeinated coffee.

Here's the deal with pairing guarana and moclobemide. Guarana is basically concentrated caffeine, and moclobemide is a reversible MAO inhibitor used to treat depression. The worry is mostly about overdoing the caffeine. When you stack a big caffeine load on top of an MAOI, blood pressure can spike, and in rare cases this could push toward a dangerous surge called a hypertensive crisis (think pounding headache, racing heart, flushing, or feeling very unwell).

I want to be honest about the evidence here: this is theoretical, based on lab studies and an old case report with a different, stronger MAOI. A normal cup of coffee or tea is unlikely to cause trouble, but concentrated guarana energy products are a different story. Please check with your pharmacist or doctor before combining them.
