CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

Nalidixic Acid falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: nalidixic acid (NegGram) is an antibiotic that can mildly slow down a liver enzyme called CYP1A2. That same enzyme is what your body uses to clear out caffeine, and guarana is basically packed with caffeine. So while you're on this antibiotic, the caffeine from guarana may hang around longer than usual and stack up in your system.
What that can feel like is more of the usual caffeine overload: jitteriness, a racing or pounding heart, trouble sleeping, or an upset stomach. The good news is this is only theoretical and rated as a minor interaction, based on lab data rather than real patient reports. Still, if you notice you feel extra wired, it's worth going easy on guarana and checking with your pharmacist or doctor.

QUINOLONE ANTIBIOTICS

Guarana contains caffeine. Theoretically, concomitant use might increase serum caffeine concentrations and the risk of adverse effects. Quinolones (also called fluoroquinolones) can decrease caffeine clearance by inhibiting the cytochrome P450 1A2 enzyme.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Nalidixic Acid (NegGram) is part of the quinolone antibiotic family, and these drugs can slow down a liver enzyme (CYP1A2) that normally clears caffeine from your body. Since guarana is basically concentrated caffeine, taking the two together means caffeine may hang around longer and build up more than usual.
What that feels like is just more of the classic caffeine effects: jitteriness, a racing or pounding heart, trouble sleeping, an upset stomach, or feeling anxious. The good news is this is rated a minor interaction and the evidence is mostly theoretical, so it's about the caffeine, not the antibiotic failing. If you notice the buzz feels stronger than expected, ease up on guarana and other caffeine, and check with your pharmacist or doctor.
