Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the simple version: this isn't about kava changing how well maraviroc fights HIV. Both kava and maraviroc are processed by your liver, and each can occasionally stress it. Maraviroc carries its own warning about possible liver trouble, and kava has been tied to rare cases of liver damage, mostly with heavy or long-term use. The worry is that stacking two liver stressors together could add up.

I'll be honest, this concern is mostly theoretical and based on scattered reports, not strong studies. Still, with an HIV medicine you want to protect, I'd keep your liver in good shape. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your pharmacist or doctor before combining them.

Maraviroc also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the quick version for Maraviroc: your body relies on a liver enzyme called CYP3A4 to clear it from your system. On paper, kava looked like it might block that enzyme, which could let maraviroc build up and possibly cause more side effects like dizziness, headache, or a cough. But that's mostly lab-dish data.

The better human research tells a calmer story. In a study where healthy people took kava daily for 28 days, it did not meaningfully change CYP3A4 activity. So a real-world interaction here is unlikely and graded as minor. That said, with an important HIV medicine you don't want surprises, and kava carries its own rare liver risk. Please run this combo past your pharmacist or doctor before adding kava.

About Maraviroc

What Is Maraviroc Used For?

Brand names: Celsentri, Selzentry

Maraviroc treats a specific type of HIV-1 infection called CCR5-tropic HIV-1. It is always used alongside other antiretroviral (anti-HIV) medicines — never alone. A special test called a tropism assay must be done before starting treatment to confirm you have the right type of HIV.

Selzentry (the brand-name oral solution and tablet) is approved for adults and pediatric patients weighing at least 2 kg. Generic maraviroc tablets are approved for adults and pediatric patients aged 2 years and older weighing at least 10 kg. Neither form is recommended for patients whose HIV is classified as dual/mixed-tropic or CXCR4-tropic.
