Acetaminophen, Caffeine, Dihydrocodeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the gist of it. The caffeine in your Panlor combo is cleared by a liver enzyme called CYP1A2. In a test tube, red clover's plant estrogens can slow that enzyme down a touch, which in theory could let caffeine linger a bit longer and bump up jittery feelings like a racing heart, restlessness, or trouble sleeping.
But here's the reassuring part: when researchers actually tested red clover capsules (a solid 60 mg of isoflavones twice daily) in real people, they saw no real change in how caffeine was handled. So this is mostly a lab curiosity, rated minor and unlikely. The dihydrocodeine and acetaminophen pieces aren't really involved here. If you tend to feel wired from caffeine, just mention it to your pharmacist or doctor.

Here's the gist for this combo pill (acetaminophen, caffeine, and dihydrocodeine). In a lab dish, red clover looked like it might slow down CYP3A4, one of the liver enzymes that helps process the dihydrocodeine and caffeine parts. If that happened in your body, levels could creep up a bit, which in theory could mean more drowsiness, dizziness, constipation, or jitteriness from the caffeine.

CAFFEINE

Red clover contains genistein. Taking genistein 1 gram daily for 14 days seems to inhibit caffeine clearance and metabolism in healthy females. However, this effect does not seem to occur with the lower amounts of genistein found in red clover. A clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine.

Here's the gist for the caffeine in your Panlor combo. In theory, the genistein found in red clover could slow how fast your body clears caffeine. If that happened, caffeine might linger a bit longer and you could feel more jittery, have a faster heartbeat, or trouble sleeping. But this is really just a lab-and-pharmacology idea, not something seen in real life at normal doses.
The reassuring part: a study using red clover standardized to 60 mg isoflavones twice daily showed no effect on caffeine at all. The slowdown only showed up with concentrated genistein (1 gram daily), far more than red clover supplements provide. So I wouldn't lose sleep over this one. If you're sensitive to caffeine or have questions, just check with your pharmacist.
