Here's something worth knowing about this combo pill. The codeine portion is a prodrug, meaning it doesn't do much on its own. Your body has to convert it into morphine using a liver enzyme called CYP2D6 for it to relieve pain. Lab studies suggest yohimbine (the active part of yohimbe) may block that same enzyme. If it does, less codeine gets turned into morphine, so you could get weaker pain relief than expected.

I'll be honest, this is theoretical and based only on test-tube research, so I wouldn't lose sleep over it. The bigger concern is yohimbe itself raising your blood pressure and heart rate. Before pairing them, run it by your pharmacist or doctor.

Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

This combination product contains aspirin, which already thins the blood a little by keeping platelets from clumping together. Lab studies suggest that yohimbine (the active piece of yohimbe) can also nudge platelets in that same anti-clumping direction. So in theory, taking them together could add up and make minor bleeding a touch more likely, like easier bruising, nosebleeds, or bleeding that takes longer to stop.

That said, this is theoretical. It comes from test-tube work, not from real patients, and we don't even know how much yohimbine your body actually absorbs from the bark. I'd be more worried about yohimbe raising your blood pressure and heart rate than about bleeding here. Still, run it by your pharmacist before adding yohimbe, especially since the codeine and carisoprodol can make you drowsy.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.
