Dolutegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yohimbe interacts with.

Theoretically, yohimbe might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yohimbe extract induces CYP3A4 enzymes.

Here's the gist with Juluca (dolutegravir and rilpivirine), your two-in-one HIV pill. The rilpivirine half is partly handled by a liver enzyme called CYP3A4. In some lab dish studies, yohimbe seemed to speed that enzyme up. If that held true in people, your body might clear rilpivirine a little faster, which could lower its level. With an HIV medication, even a modest drop in drug level is worth taking seriously, because keeping the virus suppressed depends on steady blood levels.

That said, this is only a theoretical concern from test-tube research, rated minor, and we don't have real-world cases proving it matters. Still, given yohimbe's own risks (racing heart, high blood pressure) and how important Juluca is, please check with your pharmacist or doctor before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
