CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Guaifenesin, Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Yohimbe interacts with.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo product. The hydrocodone piece is a prodrug, meaning your body uses an enzyme called CYP2D6 to turn part of it into a stronger painkiller (hydromorphone). Lab studies hint that yohimbine, the active part of yohimbe, may block that enzyme. If it does, you might actually get a bit less pain relief from the hydrocodone, since less of it gets converted.
On top of that, yohimbe and the pseudoephedrine in this product are both stimulants. Stacking them can push your blood pressure and heart rate up, leaving you jittery, anxious, or with a pounding heartbeat. Keep in mind this enzyme concern is only theoretical and from test-tube work, so don't panic. Still, please run it by your pharmacist or doctor before combining them.

Yohimbine, a constituent of yohimbe, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. Theoretically, taking yohimbe with stimulant drugs can have additive stimulant and hypertensive effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of this combo product that matters here is the pseudoephedrine, which is a decongestant that mildly stimulates your system and can nudge up your blood pressure and heart rate. Yohimbe acts in a similar stimulant-like way. Stacking the two could add up, so you might notice a racing or pounding heart, jitteriness, higher blood pressure, or trouble sleeping.
To be fair, this concern is theoretical, based on how both work rather than strong studies, so it may not cause trouble for everyone. Still, over-the-counter yohimbe doses vary a lot and can be unpredictable. The guaifenesin (a mucus thinner) and hydrocodone (a cough suppressant/opioid) aren't the main players in this particular interaction. If you have any heart issues or high blood pressure, please check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that yohimbe extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo cough and cold product has a few moving parts, but the one that matters here is hydrocodone, an opioid that your liver partly activates and clears through an enzyme called CYP3A4. Lab studies (test tube only) hint that yohimbe might speed up that enzyme. In theory, that could lower hydrocodone levels a bit and make it a touch less effective for cough or pain. Honestly, this is weak, theoretical evidence, so I wouldn't lose sleep over it.
My bigger real-world concern is the pseudoephedrine. Both pseudoephedrine and yohimbe can raise blood pressure and speed up your heart, so stacking them may make you feel jittery, anxious, or give you a pounding heartbeat. Please check with your pharmacist or doctor before pairing these.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
