Theoretically, guarana might reduce the effects of phenobarbital and increase the risk for convulsions.
Guarana contains caffeine. Animal research shows that caffeine can decrease the anticonvulsant activity of phenobarbital. The exact mechanism of this interaction is unclear.

Here's the gist with guarana and your seizure medicine. Guarana is basically a concentrated source of caffeine, and caffeine is a brain stimulant. Phenobarbital works in the opposite direction by calming overactive nerve signals to prevent seizures. So in theory, loading up on caffeine could work against that calming effect and make a seizure a little more likely.

I want to be honest about how strong this is: the concern comes mainly from animal studies, not solid human data, so it's more of a theoretical caution than a sure thing. Still, with a seizure medicine I'd rather be careful. An occasional small amount probably isn't a big deal, but a high-dose guarana energy product is a different story. Check with your pharmacist or doctor before adding it.

Here is what we know about pairing guarana with your phenytoin (this combo also includes phenobarbital). Guarana is basically a concentrated source of caffeine. In animal studies, caffeine appeared to weaken phenytoin's seizure-controlling effect, which could theoretically make a breakthrough seizure more likely. The catch is that this is based on lab and animal data, not strong human studies, so we don't fully understand the mechanism or how often it really matters in people.

Because phenytoin is a narrow-therapeutic-index drug, even small shifts can be a big deal, so I wouldn't want to brush this off entirely. A morning coffee is one thing, but loading up on guarana energy products is where I'd be more cautious. Please check with your pharmacist or doctor before adding it rather than starting or stopping anything on your own.
