Hemp and Esomeprazole, Naproxen

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Hemp interacts with whole categories of medication. Esomeprazole, Naproxen falls into 3 of those categories, so it can interact with Hemp in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 3

Why this applies to Esomeprazole, Naproxen

Esomeprazole, Naproxen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.

Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Vimovo combines naproxen (an NSAID pain reliever) with esomeprazole (a stomach acid blocker that helps protect against ulcers). The naproxen part can already thin the blood a bit and irritate the stomach lining, so bleeding is the effect we keep an eye on here. The theory is that hemp seed might also nudge your platelets to be slightly less sticky, which in theory could add to that bleeding risk.

Here's the reassuring part: this concern comes mostly from lab and animal studies. In actual human research, hemp seed oil at 2 grams a day for 12 weeks did not meaningfully affect platelets. Food amounts of hemp seeds are very unlikely to cause trouble. Still, watch for easy bruising, nosebleeds, or dark stools, and check with your pharmacist or doctor before using concentrated hemp products.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Esomeprazole, Naproxen also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that hemp induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS

Vimovo combines two medicines: esomeprazole (a stomach acid reducer) and naproxen (an anti-inflammatory pain reliever). The note on file is about a lab observation that hemp might speed up an enzyme called CYP1A2, which could in theory lower the levels of drugs that rely on it. If that happened here, you might get a little less relief from the naproxen or a bit less acid protection from the esomeprazole.

Honestly, I wouldn't lose sleep over this one. The evidence is only from test-tube and animal studies, the interaction is rated minor, and neither of these drugs depends heavily on CYP1A2. Eating hemp seeds or oil as food is very unlikely to matter. Still, if you take concentrated hemp supplements daily, mention it to your pharmacist so they can keep an eye on things.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Esomeprazole, Naproxen also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Vimovo combines two medicines, esomeprazole (a stomach acid blocker) and naproxen (an anti-inflammatory pain reliever). The concern on file is that hemp might speed up a liver enzyme called CYP3A4, which can play a role in how the body processes these drugs. If that happened, the medicines could clear out a little faster, meaning slightly weaker effects, such as less heartburn protection or less relief from pain and stiffness.

I want to be straight with you though: this is only based on lab studies, not real people, and it's rated minor. In everyday life, eating hemp seeds or hemp oil is very unlikely to throw off your Vimovo. If you use concentrated hemp supplements regularly, just mention it to your pharmacist or doctor to be safe.

From our pharmacist’s overview on the Esomeprazole, Naproxen page

“Bleeding risk is the thread running through nearly this whole page. Vimovo pairs naproxen, an anti-inflammatory that already makes stomach bleeding more likely, with esomeprazole on board to shield the gut, and most of the supplements flagged here could push that bleeding risk higher still.”

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.
