CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Fostemsavir Tromethamine
Fostemsavir Tromethamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the bottom line on pairing kava with your HIV medication Rukobia (fostemsavir). Fostemsavir is handled by a liver enzyme called CYP3A4, and the worry on paper is that kava might slow that enzyme down, which could let the drug build up a little. In theory, higher levels might mean more of fostemsavir's usual effects, like nausea or headache.
The good news is that this concern looks mostly theoretical. While early lab (test-tube) studies hinted kava might block CYP3A4, a better human study found it had no real effect on this enzyme. So a meaningful interaction here is unlikely. My bigger caution is about kava and your liver, since kava has been linked to rare liver damage. Please check with your pharmacist or doctor before adding it.
