Why this applies to Indacaterol

Indacaterol falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Indacaterol is an inhaled, long-acting bronchodilator you use to keep the airways open in COPD. It happens to be carried around the body by a transporter called P-glycoprotein (P-gp), and kava might, in theory, slow that transporter down. If P-gp were blocked, a bit more indacaterol could linger, which could nudge up its usual side effects like a fast or pounding heartbeat, shakiness, or a jittery feeling.

Here's the reassuring part: this concern comes mainly from lab dish studies, and a human study using kava found no real effect on a similar P-gp drug (digoxin). So a meaningful problem is unlikely. Still, since kava can also stress the liver, it's worth a quick chat with your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Indacaterol is a long-acting inhaled medicine that opens up the airways to make breathing easier in COPD. Part of how your body processes it involves a liver enzyme called CYP3A4, and there was a theory that kava might slow that enzyme down. If it did, indacaterol could build up a bit, which in theory might mean more of the usual side effects like a fast or pounding heartbeat, shaky hands, or a headache.

Here's the reassuring part: the better-quality human research shows kava really doesn't seem to affect CYP3A4 at all. So this concern stays mostly on paper. It's still smart to mention any kava use to your pharmacist or doctor, especially since kava can be hard on the liver.
