Why this applies to Modafinil

Modafinil falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist with kava and modafinil. Modafinil is moved around the body in part by a transporter called P-glycoprotein (P-gp). In lab dish studies, kava seemed to slow that transporter down, which in theory could let a little more modafinil hang around and nudge up its usual effects, things like jitteriness, trouble sleeping, a racing heart, or headache.

But here's the honest part: this concern comes only from test-tube work, and the research is mixed. In a real human study, kava didn't change levels of digoxin, another P-gp drug. So for most people this is more theoretical than proven. Since kava also carries its own rare liver risk, it's still worth a quick chat with your pharmacist or doctor before combining the two.

Modafinil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body uses a liver enzyme called CYP3A4 to help clear modafinil. In test-tube studies, kava looked like it might slow that enzyme down, which in theory could let modafinil build up and lead to more of its usual effects, like trouble sleeping, headache, jitteriness, a faster heartbeat, or feeling on edge.

The good news is that when this was actually tested in people taking kava for 28 days, it had no real effect on this enzyme. So this concern is mostly theoretical and unlikely to cause a problem. A bigger thing to keep in mind is that kava itself has been linked to rare liver injury, so it's worth running it past your pharmacist or doctor before combining the two.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
