Rabeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence suggests that red clover weakly inhibits CYP2C19. This interaction has not been reported in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist: your body relies on a liver enzyme called CYP2C19 to break down rabeprazole. In lab dishes, red clover appears to weakly block that same enzyme. In theory, slowing it down could let a bit more rabeprazole linger in your system, which might mean slightly stronger acid-blocking effects or minor side effects like headache, nausea, or loose stools.
The good news is this is all very theoretical. It's based only on test-tube data and has never actually been reported in real people, and rabeprazole is a fairly forgiving drug. So I wouldn't lose sleep over it. Still, if you plan to add red clover for menopause symptoms, give your pharmacist or doctor a quick heads-up first, especially since red clover has its own hormone-related cautions.

In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the short version: this is a very mild, mostly theoretical concern. In lab studies, red clover's isoflavones might slow down an enzyme called CYP2C9, which helps your body clear certain medications. Rabeprazole is partly handled by this enzyme, so in theory the levels could nudge up a little. That could mean slightly more of the usual rabeprazole effects, like headache or stomach upset, but we're talking small possibilities here.
The reassuring part is that a real human study found red clover capsules did not meaningfully change how the body processes a similar CYP2C9 drug. So for most people, this combo is unlikely to cause problems. Rabeprazole is also a forgiving medication with a wide safety margin. Still, if you take both regularly, it's worth a quick check with your pharmacist or doctor.

In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist with rabeprazole and red clover. Rabeprazole is mostly broken down without leaning heavily on the CYP3A4 enzyme, so even though lab tests suggest red clover might slow CYP3A4 a little, that pathway just isn't the main one for this acid-reducing medicine. In theory, slowing CYP3A4 could let a 3A4 drug build up, which would mean more of its effects, but rabeprazole doesn't fit neatly into that worry.
The real-world evidence is reassuring too. When researchers gave people standardized red clover and tested a true CYP3A4 drug, blood levels didn't budge. So I'd call this a very low concern. Still, if you're managing GERD or an ulcer, it's worth a quick word with your pharmacist before adding red clover, especially with hormone-sensitive conditions.

What Is Rabeprazole Used For?

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.
In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
