Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.

Here's the wrinkle with this combo. The codeine part of this medicine is actually a prodrug, meaning your body has to convert it into morphine using an enzyme called CYP2D6 before it can relieve pain. Lab studies hint that yohimbine, the active compound in yohimbe, may block that very enzyme.

So in theory, yohimbe could mean less pain relief from the codeine, not more, since less of it turns into the active form. Keep in mind this is based only on test-tube research, so it's far from certain. Honestly, the bigger worry is yohimbe itself, which can spike your blood pressure and heart rate. Please run this past your pharmacist or doctor before mixing them.

Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

This combination product pairs aspirin (a blood thinner that keeps platelets from clumping) with codeine for pain and phenobarbital. The piece that matters here is the aspirin. In lab studies, the yohimbine in yohimbe seemed to make platelets slightly less sticky too, so on paper, stacking it on top of aspirin could add to that blood-thinning effect and theoretically nudge up your odds of bruising or minor bleeding.

I'll be honest though, this is a theoretical concern based only on test-tube work, not on real patients, and it's rated minor. The bigger reason I'd flag yohimbe is its own habit of raising blood pressure and heart rate. Before adding it, please check with your pharmacist or doctor.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
