Treprostinil (Inh) falls under ANTIHYPERTENSIVE DRUGS — the group Yohimbe interacts with.
Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with pairing yohimbe and inhaled treprostinil (Tyvaso). Treprostinil works by relaxing and widening blood vessels, which helps lower the high pressure in the vessels feeding your lungs. Yohimbine, the active piece of yohimbe, tends to do the opposite in the body: it can raise blood pressure and speed up your heart rate. So on paper, yohimbe could work against what treprostinil is trying to accomplish.
To be fair, this is a theoretical interaction based on how these two behave, not something proven in studies with treprostinil specifically. Still, since you're managing a serious lung and heart condition, I'd rather you not gamble with it. Over-the-counter yohimbe products are also unpredictable in strength. Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with your inhaled treprostinil (Tyvaso). Besides relaxing blood vessels in your lungs, treprostinil also mildly thins the blood by making platelets less sticky. Yohimbe may do a little of the same thing. In lab studies, yohimbine (the active compound in yohimbe) kept platelets from clumping together. So on paper, using both could add up and nudge you toward easier bruising or bleeding.
That said, this is only a theoretical concern from test-tube work, and it hasn't shown up in real-world patient studies. I'd honestly be more worried about yohimbe's tendency to spike your blood pressure and heart rate, which isn't ideal when you're already managing pulmonary hypertension. Please run it by your pharmacist or doctor before adding yohimbe.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. What Is Treprostinil Oral Inhalation Used For?
Brand name: Tyvaso
Treprostinil oral inhalation is used to treat two serious lung conditions that raise pressure in the blood vessels of the lungs. It helps improve how much physical activity you can do.
Tyvaso inhalation solution and Tyvaso DPI are both indicated to treat pulmonary arterial hypertension (PAH; WHO Group 1) and pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3). Clinical studies establishing effectiveness in PAH primarily included patients with moderate symptoms (NYHA Class III) whose PAH was idiopathic, heritable, or related to connective tissue disease. Studies in PH-ILD primarily included patients with idiopathic interstitial pneumonia, combined pulmonary fibrosis and emphysema, or connective tissue disease-related lung disease.
