Based on the available evidence, the overall risk of a clinically significant Astragalus drug interaction is moderate, and it matters most for people taking certain closely monitored medicines. None of the known interactions are rated major, all are rated moderate, and only some are supported by human research.

Interactions deserving the most attention

The best-documented concern involves diabetes medications: human studies show astragalus may lower blood sugar, so combining them could raise the chance of blood sugar dropping too low. Because astragalus seems to stimulate the immune system, it could also work against immunosuppressants, a real issue for transplant patients whose drug effects must stay steady. Animal research also suggests astragalus acts as a mild diuretic, which in theory could slow the body's clearance of lithium and raise its levels.

Overview

Astragalus is a plant whose dried root has been used in traditional Chinese medicine for hundreds of years. The most commonly used species is Astragalus membranaceus. The plant is native to parts of Asia, including China and Mongolia, and has small yellow or purple flowers and a long, tapering root.

In traditional medicine, astragalus is often called a "tonic" or adaptogen — a term used for herbs thought to help the body resist stress and stay balanced. People most often take it to support the immune system and to help with tiredness, colds, and general wellness.

Today astragalus is sold as capsules, teas, powders, and liquid extracts. In some countries it is also given as an injection in hospital settings, but that form is not available as a dietary supplement in the United States.

How it works

Astragalus root contains several types of natural compounds that researchers have studied, including polysaccharides, saponins (such as astragalosides), and flavonoids. These are the substances most often credited with its possible effects.

In laboratory and animal studies, these compounds appear to influence parts of the immune system and may act as antioxidants, helping protect cells from a type of damage called oxidative stress. Some studies suggest possible effects on inflammation and on the heart.

It is important to remember that these mechanisms are mostly based on test-tube and animal research. What happens in a lab dish or in a mouse does not always happen in the human body, so these findings should be seen as early clues rather than proof.

Safety & precautions

For healthy adults, astragalus taken by mouth appears to be generally well tolerated for short-term use. However, long-term safety has not been well studied, and the quality of supplements can vary between brands.

People with autoimmune conditions (such as lupus, rheumatoid arthritis, or multiple sclerosis) should be cautious, because astragalus may affect immune activity. Those who have had an organ transplant or who take medicines that suppress the immune system should also avoid it unless a doctor approves.

Because reliable safety information is lacking, astragalus should be avoided during pregnancy and breastfeeding. Children and people with serious health conditions should only use it under the guidance of a healthcare professional. As with any supplement, choose products from reputable companies and tell your pharmacist or doctor what you are taking.

Side effects

When taken by mouth in usual amounts, astragalus is usually well tolerated. Some people may experience mild side effects such as an upset stomach, loose stools, or other digestive discomfort.

Allergic reactions are possible, especially in people sensitive to plants in the legume (bean) family. Stop using it and seek medical care if you have signs of an allergic reaction such as rash, itching, swelling, or trouble breathing.

More serious reactions have mainly been linked to injectable forms used in hospital settings, which are not the same as oral supplements. As with all supplements, the safest approach is to start low, watch how you feel, and check with a pharmacist if anything seems off.

Orally, astragalus has reportedly been associated with lacunar angina in one clinical trial. However, this may not have been caused by astragalus. In addition, rapid intravenous administration of astragalus has resulted in temporary palpitations.
