Effient falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the thing about Effient (prasugrel): it's a prodrug, meaning your body has to activate it before it can work to thin your blood. That activation happens mostly through enzymes called CYP3A4 and CYP2B6, not really CYP2C9. So the lab finding that Ceylon Leadwort's plumbagin blocks CYP2C9 doesn't line up well with how Effient is processed.

Bottom line, I wouldn't expect a meaningful interaction here, and remember this is only test-tube data with nothing seen in real people. Still, because Effient already raises bleeding risk, it's worth a quick chat with your pharmacist or doctor before adding any new herbal product, just to play it safe.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's something worth knowing about Effient (prasugrel): it's a prodrug, meaning it has to be switched on by your body before it can thin your blood. That activation step uses several enzymes, including CYP3A4. Lab studies suggest plumbagin, a compound in Ceylon Leadwort, can slow CYP3A4 down. In theory, that could mean a bit less of Effient gets activated, which might make it slightly less effective at preventing clots, the very thing it's protecting you from after a heart attack or stent.

I want to be honest about the evidence here: this is only test-tube and animal data, never confirmed in real people, so it's a theoretical concern, not a proven one. Effient also relies on more than one enzyme, which softens the worry. Still, since this drug guards against serious clotting, please check with your pharmacist or doctor before combining the two.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Effient (prasugrel) already does a powerful job of keeping your platelets from sticking together, which is exactly why your doctor paired it with aspirin after your heart procedure. The concern here is simple: Ceylon Leadwort may have its own mild blood-thinning action. Stack that on top of Effient and you could nudge your bleeding risk higher than intended.

What that might look like is easy bruising, nosebleeds, bleeding gums, or cuts that take longer to stop. I want to be honest, though: this is based only on animal and lab research, not human studies, so we don't really know how strong the effect is in people. Because Effient is already a strong antiplatelet drug, I'd play it safe and check with your pharmacist or doctor before adding this supplement.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2B6. Drugs that are metabolized by CYP2B6 include ketamine (Ketalar), phenobarbital, orphenadrine (Norflex), secobarbital (Seconal), and dexamethasone (Decadron).

Here's the honest picture with Effient (prasugrel) and Ceylon Leadwort. The concern on file is that plumbagin, a compound in the plant, can block a liver enzyme called CYP2B6. But this is based only on lab (test tube) research, and it has never actually been reported in people. On top of that, Effient mostly relies on different enzymes (CYP3A4 and CYP2B6 do play a small part, but it isn't a major CYP2B6 drug). So this is a theoretical interaction, not something well documented.

Because Effient is a blood thinner that prevents clots, the thing to watch in general is bleeding, like easy bruising, nosebleeds, or bleeding that won't stop. I wouldn't lose sleep over food-level exposure, but supplement doses are a different story. Please run it by your pharmacist or doctor before adding Ceylon Leadwort.

What Is Prasugrel Used For?

Brand name: Prasugrel

Prasugrel tablets (brand names Effient and Prasugrel) are used to lower the risk of dangerous blood clots — including clots that can block a stent — in people with acute coronary syndrome (ACS), which includes unstable angina (chest pain at rest) and heart attacks (both NSTEMI and STEMI types). It is used specifically in patients who are being treated with a procedure called percutaneous coronary intervention (PCI), in which a catheter is used to open a blocked coronary artery, often with a stent.

Effient has been shown to reduce the combined rate of cardiovascular death, nonfatal heart attack, or nonfatal stroke compared to clopidogrel — with the benefit driven mainly by fewer heart attacks.
