Theoretically, guarana might reduce the effects of phenobarbital and increase the risk for convulsions.
Guarana contains caffeine. Animal research shows that caffeine can decrease the anticonvulsant activity of phenobarbital. The exact mechanism of this interaction is unclear.

Here's the simple version. Phenobarbital is a barbiturate that calms down overactive electrical activity in the brain, which is part of how it helps prevent seizures and promote sleep. Guarana is basically a concentrated source of caffeine, and caffeine is a stimulant that works in the opposite direction by waking the brain up.

Because of that tug-of-war, the worry is that guarana could blunt some of phenobarbital's calming, seizure-protecting effect, which might raise the chance of a seizure or make it harder to wind down at night. I'll be honest with you though: this concern comes mostly from animal research, not strong human studies, so it's more of a theoretical caution. A small amount of caffeine is unlikely to be a big deal, but regular high-dose guarana energy products are worth running past your pharmacist or doctor first.
