Why this applies to Vinblastine

Vinblastine falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist. Your body uses a kind of cellular pump called P-glycoprotein to push vinblastine out of cells, which helps keep its levels in check. Lab and test-tube studies hint that kava might slow this pump down. If that happened in real life, more vinblastine could linger in your system, which could mean stronger side effects like low blood counts (more infections, bruising, or bleeding), nausea, constipation, or nerve tingling. Vinblastine is not a prodrug, so blocking the pump would tend to raise its effect, not lower it.

That said, the evidence here is weak, mostly test-tube work, and a human study with another pump drug (digoxin) showed no real change. Still, chemo leaves little room for surprises, so please clear any kava with your oncology team first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Vinblastine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body clears vinblastine using a liver enzyme called CYP3A4, and there was a worry that kava might slow that enzyme down. If it did, vinblastine could build up and lead to stronger side effects like low blood counts, infections, nerve tingling, or constipation. The good news is that the better-quality human research doesn't back this up. In a study where healthy volunteers took kava for 28 days, it did not actually change CYP3A4 activity.

So this interaction is rated minor and unlikely. Still, vinblastine is a powerful chemo drug, and kava carries its own risk of liver harm. Please loop in your oncology team or pharmacist before adding kava.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

What Is Vinblastine Used For?

Brand name: Velban

Vinblastine Sulfate Injection is used in the treatment of a range of cancers. Conditions that tend to respond well include generalized Hodgkin's disease, lymphocytic lymphoma, histiocytic lymphoma, advanced mycosis fungoides (a type of skin lymphoma), advanced testicular cancer, Kaposi's sarcoma, and Letterer-Siwe disease (a rare immune condition also called histiocytosis X).

It may also be used for cancers that respond less reliably, such as choriocarcinoma resistant to other chemotherapy drugs, and breast cancer that has not responded to hormonal therapy. Vinblastine is often given as part of a combination chemotherapy regimen alongside other cancer medicines to improve response rates.
