The piece that matters here is the codeine in this combination product. Codeine is actually a prodrug, meaning your body has to convert it into morphine (using the liver enzyme CYP2D6) before it relieves much pain. Lab studies hint that yohimbine, the active part of yohimbe, can slow that same enzyme down. In theory, that could mean less codeine gets turned into morphine, so you might get weaker pain relief.

Honestly, this is based on test-tube research only, so it's far from certain. The bigger real-world worry is that yohimbe itself can spike your blood pressure and heart rate. Don't stop or change anything on your own, but do run it by your pharmacist or doctor first.

Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.

Here's the angle on this combo: your medicine already contains aspirin, which thins the blood a bit by making platelets less sticky. Yohimbe (through its active compound yohimbine) may nudge platelets in the same direction. So in theory, pairing them could add up and raise your chances of bruising, nosebleeds, or stomach bleeding, which aspirin alone can already cause.

That said, this is mostly lab-dish science, not something seen in real patients, so I wouldn't lose sleep over it. The bigger worry with yohimbe is its own punch on the heart, like high blood pressure and a racing pulse. Don't change anything on your own. Run it by your pharmacist or doctor first, especially since over-the-counter yohimbe products are wildly inconsistent.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate.
