Why this applies to Zanubrutinib

Zanubrutinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist for Brukinsa (zanubrutinib). Your body uses a kind of cellular bouncer called P-glycoprotein (P-gp) to help move this medication along and limit how much builds up. In test-tube studies, kava appeared to slow that bouncer down, which in theory could let more zanubrutinib hang around and raise the odds of side effects like bruising, bleeding, infections, low blood counts, or an irregular heartbeat.

But the evidence here is thin. A human study using a different P-gp drug (digoxin) found no real effect from kava, so this concern is mostly lab-based, not proven in people. Still, kava carries its own rare liver risk, which matters during cancer treatment. Please check with your oncology pharmacist or doctor before combining them.

Zanubrutinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body uses an enzyme called CYP3A4 to break down zanubrutinib (Brukinsa). The thought was that kava might block that enzyme, which could let the drug build up and raise the odds of side effects like bruising, bleeding, low blood counts, or infections. But when researchers actually tested this in people, kava at a typical dose didn't meaningfully change CYP3A4 activity. The lab-dish results that raised the worry didn't hold up in real life.

So this interaction is rated minor and unlikely. The bigger concern with kava is its rare link to liver damage, which matters more when you're on cancer therapy. Please loop in your oncologist or pharmacist before adding kava.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. About Zanubrutinib

What Is Zanubrutinib Used For?

Brand name: Brukinsa

Brukinsa (zanubrutinib) is used to treat adult patients with certain blood cancers. It treats mantle cell lymphoma (MCL) in patients who have had at least one prior therapy, Waldenström's macroglobulinemia (WM), relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 treatment, and chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Brukinsa is also used — in combination with another medicine called obinutuzumab — to treat relapsed or refractory follicular lymphoma (FL) in adults who have already received two or more lines of treatment. Several of these uses were granted accelerated FDA approval based on response rates, meaning confirmatory trials are still ongoing.
