Lansoprazole, Amoxicillin, Clarithromycin falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the deal with guarana alongside your Prevpac (lansoprazole, amoxicillin, and clarithromycin). Guarana is basically a concentrated source of caffeine, and your body clears caffeine using a liver enzyme called CYP1A2. One of the ingredients in this pack, clarithromycin, can mildly slow that enzyme down. In theory, that means caffeine could hang around in your system a bit longer, so you might feel more wired than usual: jitteriness, a racing heart, trouble sleeping, or an upset stomach.

That said, this is rated minor and is based only on lab-type evidence, not real patient reports, so for most people it's a small concern. Still, since this combo can already cause nausea and a funny taste, I'd keep guarana modest and check with your pharmacist if you feel overly jittery.
