Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the main thing to know about pairing kava with trazodone: this isn't really about kava changing how your body processes the medicine. It's about the liver itself. Kava has been tied to rare cases of liver injury, mostly with heavy or long-term use, and trazodone is one of those medicines that can occasionally stress the liver too. Stack two liver concerns together and you may bump up the overall risk.

To be honest, this is a theoretical caution built on anecdotal reports, not strong proof. Still, I'd take it seriously. Watch for things like yellowing skin or eyes, dark urine, belly pain on the right side, or unusual tiredness. Trazodone can also make you quite drowsy, and kava's calming effect may add to that. Please check with your pharmacist or doctor before combining them.

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the gist with kava and trazodone. On paper, kava might block an enzyme called CYP2D6, which is one of the pathways your body uses to clear trazodone. If that blocking happened, trazodone could build up a bit, possibly leaving you more drowsy, dizzy, or lightheaded than usual. But here's the reassuring part: while test-tube studies hinted at this, actual human studies showed kava did not meaningfully change how people process CYP2D6 medications. So this interaction is considered minor and unlikely.

My bigger concern is overlap, not metabolism. Both kava and trazodone can make you sleepy and relaxed, so together they may add up. Kava can also stress the liver. Please run this combo by your pharmacist or doctor before mixing them.

Trazodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the deal with kava and trazodone. Your body clears trazodone using a liver enzyme called CYP3A4. Lab tests once hinted that kava might block that enzyme, which in theory could let trazodone build up and lead to more of its usual effects, like drowsiness, dizziness, or feeling foggy. But when researchers actually tested this in people taking kava for 28 days, it had no real effect on CYP3A4. So this particular interaction is rated minor and unlikely.

That said, both kava and trazodone can make you sleepy on their own, so combining them may add to that drowsy feeling. Kava has also been tied to rare liver problems, which matters since trazodone is processed by the liver. I'd run this combo by your pharmacist or doctor first.
