Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the thing with kava and amiodarone: both can be hard on your liver. Amiodarone is already well known for occasionally raising liver enzymes and, in rare cases, causing more serious liver injury. Kava has been tied to over 100 reports of liver problems, mostly with heavy or long-term use. So the worry is simple addition, two things that stress the liver at the same time.

To be fair, this is a theoretical, anecdotal concern, not a proven, predictable reaction. Still, since amiodarone needs regular liver monitoring anyway, I'd lean cautious. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your doctor or pharmacist before combining them.

Amiodarone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with amiodarone. Your body uses a liver enzyme called CYP3A4 to help process amiodarone, and there was a thought that kava might block that enzyme and let amiodarone build up. In theory, higher amiodarone levels could mean more side effects like a very slow heartbeat, dizziness, or thyroid and lung problems over time. But the better-quality human study we have showed kava did not actually change CYP3A4 activity, so this worry mostly didn't hold up.

The bigger concern with this pair is your liver. Both kava and amiodarone can stress the liver on their own, so stacking them isn't something I'd do casually. Please check with your pharmacist or doctor before adding kava, since amiodarone is a powerful heart drug.

What Is Amiodarone Used For?

Amiodarone is prescribed for adults with serious, life-threatening heart-rhythm problems that haven't responded to other medications. Specifically, it's used for two dangerous arrhythmias (abnormal heartbeats): recurrent ventricular fibrillation (a chaotic, potentially fatal heart rhythm) and recurrent ventricular tachycardia (a dangerously fast heart rhythm that causes unstable blood pressure).

Because amiodarone comes with significant risks, doctors reserve it for situations where other options haven't worked or can't be tolerated. It is not a first-choice medication — it's a last-resort tool for serious, life-threatening conditions.
