Bumetanide (IV) falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the short version: Danshen may gently lower blood pressure on its own, and bumetanide (Bumex) already pulls extra fluid and salt out through your urine, which can drop your blood pressure too. Stack the two and the effects could add up, so you might feel lightheaded, dizzy, or wobbly when you stand up, especially if you're also a bit dehydrated from the water pill.

I want to be honest about the evidence, though. This warning comes from lab and animal studies, not solid human trials, so it's more of a theoretical caution than a proven problem. Bumetanide works directly on your kidneys and isn't a prodrug, so Danshen isn't expected to change how it's activated. Still, please check with your pharmacist or doctor before combining them.

What Is Bumetanide Used For?

Brand name: Bumex

Bumetanide treats edema — excess fluid that builds up in the body — caused by congestive heart failure, hepatic (liver) disease, and renal (kidney) disease, including a condition called nephrotic syndrome. This applies to the oral tablets (bumetanide and Bumex), the injection (Bumetanide Injection), and the nasal spray.

Enbumyst (bumetanide nasal spray) is specifically approved for these same uses in adults. Enbumyst is not intended for long-term use and should be switched to an oral diuretic as soon as practical.
