Finasteride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the short version: your body uses a liver and gut enzyme called CYP3A4 to help break down finasteride. Danshen can nudge that enzyme in different directions. Taken steadily for a week or two, it tends to speed CYP3A4 up, which could clear finasteride a little faster and make it work slightly less well for your prostate symptoms or hair loss. Oddly, a single dose seems to do the reverse and raise levels for a short time.

The good news is that finasteride is not a narrow-margin drug, and this evidence comes from a small study using a different medicine, so the real-world effect is likely modest. Still, before combining them, check with your pharmacist or doctor so they can keep an eye on things.

About Finasteride

What Is Finasteride Used For?

Brand names: Propecia, Proscar

Finasteride tablets (including the brand Proscar) are used in men with an enlarged prostate (benign prostatic hyperplasia, or BPH) to improve urinary symptoms, lower the risk of acute urinary retention (a sudden inability to urinate), and reduce the need for prostate surgery. Finasteride tablets may also be used alongside the alpha-blocker doxazosin to slow the progression of BPH symptoms.

Propecia (and finasteride 1 mg coated tablets) is indicated for the treatment of male pattern hair loss (androgenetic alopecia) in men only. Propecia is not approved for use in women, and its effectiveness for hair loss at the temples has not been established. Finasteride is not approved for the prevention of prostate cancer.
