Fostamatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's something worth knowing if you're taking Tavalisse (fostamatinib) for your low platelets. Your body actually converts fostamatinib into its active form (R406), and that active piece is broken down by an enzyme called CYP3A4. Danshen can nudge this enzyme in different directions. Taken steadily for a week or two, danshen tends to speed up CYP3A4 in the gut, which could clear the active drug faster and make it work less well, meaning your platelet counts may not improve as expected.

To complicate things, a single dose of danshen seems to do the opposite and raise drug levels, which could mean more side effects like diarrhea, high blood pressure, or nausea. The evidence here comes from a study using a different drug, so it's suggestive rather than certain. Please check with your pharmacist or doctor before combining them.

What Is Fostamatinib Used For?

Brand name: Tavalisse

Tavalisse (fostamatinib) is used to treat thrombocytopenia — dangerously low platelet counts — in adults with chronic immune thrombocytopenia (ITP). It is specifically for people whose platelet counts have not improved enough with at least one earlier ITP treatment.

The goal is to raise platelet counts high enough to reduce the risk of serious bleeding. If platelet counts do not respond sufficiently after 12 weeks, the medication is typically stopped.
