Why this applies to Nirmatrelvir, Ritonavir

Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with Paxlovid: the ritonavir part is there on purpose to block the CYP3A4 enzyme, which keeps blood levels of the antiviral (nirmatrelvir) high enough to fight COVID. Danshen muddies that picture. Studies suggest that taking danshen for a couple of weeks can actually speed up CYP3A4 in your gut, which in theory could lower the levels of a CYP3A4 medicine and make it less effective. Interestingly, a single dose of danshen seems to do the opposite and raise levels.

The evidence here comes mostly from a midazolam study, not from Paxlovid itself, so this is somewhat indirect. Since Paxlovid is only a short 5-day course, my honest advice is to simply hold off on danshen during treatment and check with your pharmacist or doctor first.
