Ombitasvir, Paritaprevir Ritonavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist: this hepatitis C combination (Technivie) is partly handled by a transporter in your body called p-glycoprotein, which acts like a doorman moving the drug in and out of cells. Danshen may slow that doorman down. In a small human study, danshen raised levels of another p-glycoprotein drug by roughly a third, so it's reasonable to think it could nudge up the levels of this medicine too.

If that happens, you might notice more of the usual side effects, like nausea, tiredness, or trouble sleeping. The evidence here is limited and the change is likely modest, so I don't want you to panic. But because you're treating hepatitis C and your liver is already involved, please run the danshen idea past your pharmacist or doctor before starting it.

Ombitasvir, Paritaprevir Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the concern with pairing Danshen and Technivie. Part of this hepatitis C combo relies on the liver enzyme CYP3A4, and the ritonavir piece is actually included to block that enzyme so paritaprevir stays at strong, effective levels. Danshen can mess with this balance. Taken for a week or two, it seems to speed up CYP3A4 in the gut, which could lower the amount of medicine in your body and risk the treatment not fully clearing the virus.

The flip side is that a single dose appears to do the opposite and raise drug levels. Either way, this evidence is preliminary and comes from healthy volunteers, not hepatitis C patients. Because finishing this regimen correctly really matters, please check with your pharmacist or doctor before adding Danshen.

What Is Ombitasvir, Paritaprevir, and Ritonavir Used For?

Brand name: Technivie

  • The combination of ombitasvir, paritaprevir, and ritonavir is usually used in combination with ribavirin (Copegus, Rebetol), but sometimes it is used alone to treat a certain type of chronic (long-term) hepatitis C infection (swelling of the liver caused by a virus). Ombitasvir is a hepatitis C virus (HCV) NS5A inhibitor. It works by stopping the virus that causes hepatitis C from spreading inside the body. Paritaprevir is a protease inhibitor. It works by decreasing the amount of HCV in the body. Ritonavir is a protease inhibitor. It helps to increase the amount of paritaprevir in the body so that the medication will have a greater effect. It is not known if ombitasvir, paritaprevir, or ritonavir prevent the spread of hepatitis C to other people.
