Sacituzumab Govitecan-Hziy falls under GLUCURONIDATED DRUGS — the group Danshen interacts with.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the connection with Trodelvy. After this medicine releases its chemotherapy payload (a compound called SN-38), your liver clears that active piece partly by attaching a sugar molecule to it, a process called glucuronidation. Danshen has been shown in lab dishes to tug on those same glucuronidation enzymes, sometimes blocking them and sometimes ramping them up. If the enzymes are slowed, SN-38 could linger longer, which in theory might worsen typical Trodelvy side effects like low blood counts, nausea, and significant diarrhea.

I want to be honest about how shaky this is: it comes only from test-tube and animal work, and it has never actually been reported in people. Still, because Trodelvy is a serious cancer drug, please loop in your oncology team or pharmacist before adding Danshen. Don't change anything on your own.

  • Sacituzumab govitecan-hziy is used to treat ertain types of breast cancer in adults that has spread to other parts of the body or cannot be removed by surgery and who have already been treated with at least two other chemotherapy medications. Sacituzumab govitecan-hziy is also used to treat certain types of urothelial cancer (cancer of the lining of the bladder and other parts of the urinary tract) in adults that has spread to other parts of the body or cannot be removed by surgery and who have already been treated with at least two other chemotherapy medications. Sacituzumab govitecan-hziy is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
