Why this applies to Vincristine Sulfate

Vincristine Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with Danshen and vincristine. Vincristine is a chemotherapy drug that your liver and gut clear using an enzyme called CYP3A4, and it is not a prodrug, so it works as given. The studies we have suggest Danshen can change how that enzyme behaves. With regular use over a week or two, Danshen seems to speed CYP3A4 up, which could clear vincristine faster and possibly weaken the treatment you actually need. But a single dose did the opposite and raised drug levels, which could mean more side effects like nerve tingling, numbness, constipation, or low blood counts.

Because the effect can swing both ways and this is a cancer medicine, please don't add Danshen without first checking with your oncology team or pharmacist.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the situation with Danshen and vincristine. Vincristine is a strong chemotherapy drug, and your body relies on a pump called P-glycoprotein to help move it out of cells and clear it. Danshen appears to slow that pump down. In one study in healthy people, danshen raised the levels of another P-glycoprotein drug by about 30 to 40 percent. If the same thing happens with vincristine, more of the drug could build up in your system.

With vincristine, higher levels can mean more of its known side effects, like nerve problems (tingling, numbness, weakness), constipation, and low blood counts. The evidence here is limited, but because vincristine is a potent drug with a narrow safety margin, please talk with your oncologist or pharmacist before combining the two. Don't change anything on your own.
