Cariprazine Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your body relies on a liver enzyme called CYP3A4 to break down cariprazine (Vraylar). The interesting thing about danshen is that it seems to work both ways depending on how long you take it. In a small study, taking danshen for about 10 to 14 days actually sped up this enzyme, which could lower your cariprazine levels over time and make it less effective at controlling your mood or symptoms. A single dose, on the other hand, did the opposite and briefly raised levels.

So with steady danshen use, you might notice cariprazine not working as well, while a sudden change could lead to more side effects like restlessness, trouble sleeping, or muscle stiffness. The evidence here is fairly early, so please check with your pharmacist or doctor before mixing the two.

What Is Cariprazine Used For?

Brand name: Vraylar

Vraylar (cariprazine) is approved to treat schizophrenia in adults and in adolescents aged 13 and older. It is also used for the acute treatment of manic or mixed episodes associated with bipolar I disorder in adults and in children aged 10 and older.

In adults, Vraylar is additionally approved to treat depressive episodes associated with bipolar I disorder (bipolar depression) and as an add-on (adjunctive) therapy alongside antidepressants for major depressive disorder (MDD).
