Hydrochlorothiazide, Propranolol falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Inderide combines two blood pressure medicines: hydrochlorothiazide (a water pill) and propranolol (a beta blocker). Both already lower your blood pressure. Danshen, in animal studies, seems to relax blood vessels and gently push blood pressure down on its own. So the worry here is simple addition: stacking Danshen on top of your medicine could drop your blood pressure a little too far.

If that happened, you might notice dizziness, lightheadedness, feeling faint when standing up, or unusual tiredness. I want to be honest though, this concern comes mainly from lab and animal research, not solid human studies, so it is more theoretical than proven. Danshen can also affect blood clotting, which is its own separate caution. Before adding it, please check with your pharmacist or doctor.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Hydrochlorothiazide, Propranolol also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

This combo pill pairs a water pill (hydrochlorothiazide) with a beta-blocker called propranolol. The piece that matters here is the propranolol, which your liver partly clears using an enzyme known as CYP1A2. In test-tube studies, Danshen can slow that enzyme down. If that happened in the body, propranolol could build up a bit, which might mean more of its usual effects: a slower heart rate, lower blood pressure, dizziness, or tiredness.

The important catch: this is only theoretical. In actual human testing with another CYP1A2 drug, Danshen did not produce this effect. So I would not panic, but since Danshen may also thin the blood, it is worth a quick chat with your pharmacist or doctor before combining them.
