Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: Ritonavir (Norvir) is cleared in part by an enzyme called CYP3A4, and Danshen can nudge that enzyme in either direction. The research we have is small, but it suggests that taking Danshen for a stretch of time (about 10 to 14 days) tends to speed up CYP3A4 in your gut, which could lower the amount of ritonavir reaching your blood. A single dose seems to do the opposite and bump levels up.

Why this matters with HIV medicine: ritonavir levels need to stay steady, both for itself and for the other drugs it's often paired with. Lower levels could weaken treatment, while higher levels could mean more nausea, diarrhea, or tingling. Please don't start or stop anything on your own. Check with your pharmacist or HIV provider first.

What Is Ritonavir Used For?

Brand name: Norvir

Ritonavir (brand name Norvir) is used together with other HIV medicines to treat HIV-1 infection in adults and children older than one month of age. It is always taken as part of a combination regimen — never on its own.

Norvir oral powder is specifically indicated for pediatric patients with HIV-1 infection. Norvir tablets and oral solution are indicated for both adults and pediatric patients with HIV-1 infection. Ritonavir tablets (generic) are also indicated for the treatment of HIV-1 infection in combination with other antiretroviral agents.
