Saquinavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for saquinavir (Invirase): your body uses a protein pump called P-glycoprotein to move this drug around and help clear it. Danshen appears to slow that pump down. In a study with healthy volunteers, danshen pushed levels of a similar pump-handled drug up by roughly 30 to 37 percent. So with saquinavir, the worry is that more of it could build up in your blood.

For saquinavir, higher levels could mean more side effects like nausea, diarrhea, belly upset, or changes in how your heart's rhythm reads on an ECG. This is moderate, possible, and based on lower-quality evidence, so it isn't a sure thing. Still, since keeping HIV medicines steady really matters, please check with your pharmacist or doctor before combining the two. Don't stop anything on your own.

Saquinavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing about Saquinavir (Invirase): your body clears it using an enzyme called CYP3A4, found in your gut and liver. Saquinavir is not a prodrug, so it works as-is. In a small clinical study, taking Danshen for 10 to 14 days revved up that enzyme, which could clear Saquinavir from your body faster and lower its levels.

For an HIV medicine, that matters. Lower drug levels can mean the virus isn't kept in check, and could even lead to resistance. Interestingly, a single dose of Danshen did the opposite and raised levels. The evidence is limited, but because keeping HIV suppressed is so important, please talk with your pharmacist or doctor before adding Danshen.

What Is Saquinavir Used For?

  • Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
