Why this applies to Siponimod

Siponimod falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist for Siponimod (Mayzent): your body clears it partly through a liver enzyme called CYP2C9. In lab studies, certain compounds in Danshen appear to slow that same enzyme down. If that happened in real life, Siponimod could build up in your system, and stronger blood levels could mean more of its known effects, like a slower heart rate (especially when you first start it), a higher chance of infections, headaches, or changes in liver tests or blood pressure.

The honest caveat: this is only seen in test-tube and animal research, not in people, so it's theoretical for now. Still, Siponimod is a drug where dosing matters, so please check with your pharmacist or doctor before pairing it with Danshen rather than changing anything on your own.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the connection worth knowing: Siponimod (Mayzent) is cleared by an enzyme called CYP3A4, and Danshen can mess with that enzyme. The tricky part is that Danshen seems to push it in two directions. Taken steadily for a week or two, Danshen appears to speed up CYP3A4, which could lower your siponimod levels and make it work less well at controlling your MS. But a single dose did the opposite in studies, briefly raising drug levels.

Because siponimod's job is to quietly hold your immune system in check, less of it could mean weaker protection against flare-ups, while too much could add to side effects like slowed heart rate or infections. The evidence here is early and modest, not alarming, but please don't start or stop anything on your own. Run it by your pharmacist or neurologist first.

Mayzent (siponimod) is approved for adults with relapsing forms of multiple sclerosis (MS). This includes three specific types: clinically isolated syndrome (a first episode of MS-like symptoms), relapsing-remitting MS (periods of attacks followed by recovery), and active secondary progressive MS (a phase where the disease gradually worsens but still has active relapses).

It is taken by mouth once daily and is intended as a long-term treatment to reduce relapses and slow disease progression.
