Why this applies to Tasimelteon

Tasimelteon falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist with Danshen and Hetlioz (tasimelteon). Your liver uses an enzyme called CYP1A2 to break tasimelteon down. In lab dish studies, Danshen seems to slow that enzyme. If that happened in your body, tasimelteon could clear a little slower and build up, which might mean stronger or longer next-day effects like grogginess, sleepiness, headache, or odd dreams. Tasimelteon is not a prodrug, so more of it in your system means a bigger effect, not a smaller one.

The good news: this is only theoretical. When researchers tested Danshen against a similar CYP1A2 drug in real people, they didn't see a meaningful change. So I wouldn't panic, but I'd still mention it to your pharmacist or doctor before combining them, especially since Danshen can also affect bleeding.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for Hetlioz (tasimelteon): your body relies on an enzyme called CYP3A4 to break it down, and Danshen can nudge that enzyme in either direction. When taken steadily for a week or two, Danshen seems to speed up CYP3A4, which could clear tasimelteon out faster and make it work less well. In plain terms, you might find your sleep timing slips back off track or the medicine just doesn't help as much. Oddly, a single dose of Danshen does the opposite and can raise levels briefly, possibly leaving you groggier than usual.

This is based on early research using a different drug (midazolam), so the effect on tasimelteon specifically isn't proven. Still, since timing matters so much for this medicine, please check with your pharmacist or doctor before pairing them.

What Is Tasimelteon Used For?

Brand names: Hetlioz, Hetlioz Lq

Tasimelteon capsules are approved to treat Non-24-Hour Sleep-Wake Disorder (Non-24) in adults. Non-24 is a condition where the body's internal clock does not align with the normal 24-hour day, causing sleep and wake times to shift continuously.

Hetlioz capsules are also approved to treat nighttime sleep disturbances in Smith-Magenis Syndrome (SMS) in patients 16 years of age and older. Hetlioz LQ oral suspension is approved specifically for nighttime sleep disturbances in SMS in pediatric patients ages 3 to 15 years.
