Why this applies to Voriconazole

Voriconazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist with Danshen and voriconazole (Vfend). Your body relies on a liver enzyme called CYP2C9 to help break voriconazole down and clear it out. Lab studies suggest compounds in Danshen can slow that enzyme. If that happened in real life, voriconazole could build up a bit higher than expected, which might mean more side effects like blurry or altered vision, nausea, headache, or changes in liver tests.

I'll be honest though, this is theoretical right now. It comes only from test-tube research and hasn't actually been reported in people taking these two together. Voriconazole is a potent drug with a tricky dose, so I wouldn't combine them on your own. Just run it by your doctor or pharmacist first.

Voriconazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with Danshen and voriconazole. Your body partly clears voriconazole using an enzyme called CYP3A4, and this herb can nudge that enzyme. The catch is that Danshen works two ways: taken steadily for a week or two, it seems to speed CYP3A4 up, which could lower your voriconazole levels. A single dose may do the opposite and raise them, at least briefly.

Why this matters: voriconazole isn't a prodrug, so it needs to stay in a fairly narrow range to actually kill the fungus. Lower levels could let a serious infection keep growing, while higher levels can bring on side effects like blurred vision, light sensitivity, confusion, or liver strain. The human evidence here is early and modest, but with a drug this important I'd skip Danshen unless your doctor or pharmacist clears it first.

About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
